Dr. Anne Hollingsworth, D.O., FACOI, an internal medicine physician, has been practicing medicine since completing her medical education at Michigan State University College of Osteopathic Medicine, and an internal medicine residency at Oakland General Hospital in Michigan.
Board-certified in internal medicine, Hollingsworth’s areas of interest include both general and preventive medicine.
Hollingsworth’s patients are able to receive a range of treatment options for conditions such as diabetes, heart disease, hypertension, obesity, chronic obstructive pulmonary disease, and more.
Over the years, Hollingsworth’s work has also led her to cultivate a special interest in women’s health, including the health issues many women face and the importance of wellness education to prevent those issues.

Dr. Asma Aziz, M.D. practices family medicine and is board-certified by the American Board of Family Medicine.
After graduating from Fatima Jinnah Medical University in Lahore, Pakistan, Aziz then completed a family medicine residency at Henry Ford Allegiance Health in Jackson, Michigan.
Aziz is interested in treatment and prevention strategies for conditions like women’s health, chronic disease management, diabetes, hypertension, geriatric medicine, pediatrics, and wellness and lifestyle management.
Aziz cares for patients of all ages through her primary care location in La Porte, including via childhood vaccines, annual physicals, and well-child check-ups for acute or chronic illnesses.

Dr. Sarah Polistico, M.D. is a pediatric physician who specializes in childhood growth and development, age-appropriate screenings, childhood fitness and nutrition, and prevention, early detection, and management of pediatric illnesses.
After earning her medical degree from the University of Santo Tomas, Polistico then completed her pediatric residency at Makati Medical Center in the Philippines and St. Barnabas Hospital in New York.
