Effective Monday November 8, 2021, Methodist Hospitals will offer the Pfizer COVID-19 vaccine for children ages 5-11 at its Northlake and Southlake campuses:

Southlake Campus Vaccine Clinic
Pavilion B conference room
200 E. 89th Ave, Merrillville
Monday – Friday 9:00 a.m. – 3:00 p.m.
Walk-ins are accepted, but appointments are preferred. Appointments can be made at ourshot.in.gov. Methodist Hospitals also continues to offer the Pfizer vaccine for children 12-17 and the Pfizer and Moderna vaccines for adults 18 and older.
